Patient Information
Download Intake Form📅 Booking an Appointment
All appointments are by prior arrangement only. To make a booking:
-
Ask your GP, optometrist, or another specialist to provide a referral
-
Or contact us directly to arrange a private consultation (a referral is still needed to access Medicare rebates)
🧾 What to Bring
For your appointment, please bring:
-
Your referral letter (if applicable)
-
Your Medicare card and any private health insurance details
-
A list of current medications
-
Any glasses you use (especially driving glasses)
-
Results of previous eye tests or scans (if necessary)
Please note: For most appointments, we use drops to dilate your pupils. This allows us to examine the back of your eyes in detail. These drops can make your vision blurry and sensitive to light for a few hours, so we recommend bringing a driver and some sunglasses.
